About

A Coach Who Trains Dogs—and Teaches Their People

AKC Canine Good Citizen Evaluator B.S. Biological Sciences, Animal Biology 7+ Years Professional Experience

Hi! I'm Brittany, and I'm so glad you're here!

I got my start training dogs about seven years ago while working for a local dog training company. I began as a dog trainer and eventually worked my way up to General Manager, where I gained experience overseeing nearly every aspect of dog care.

From a young age, I always knew I wanted to work with animals. In 2019, I graduated with my bachelor's degree in Biological Sciences, with a focus in Animal Biology. I have always been passionate about the rescue and rehabilitation of animals, and that passion ultimately led me to dog training.

I also have a background in teaching and education, which is something I get to utilize every day when working with dog owners. One of my favorite parts of my job is seeing an owner get excited about their dog's progress and watching that moment when something finally “clicks” for a dog.

Over the years, I have had the opportunity to work with dogs of all ages, breeds, and temperaments, and in a wide variety of situations. My experience extends far beyond traditional obedience training. I have worked with deaf and blind dogs, two- and three-legged dogs, rescue dogs, and dogs with unique behavioral needs. I also have extensive experience with therapy and service dog training, and I am an AKC Canine Good Citizen Evaluator.

“I believe there is no one-size-fits-all approach to dog training. Every dog is an individual, and every household is different.”

My training approach is tailored to the dog in front of me and the people who love them. I take the time to understand your goals, your challenges, and what will realistically work for your lifestyle, so that training becomes something you can continue to build on long after our sessions together.

I also believe that dog training should be a relationship, not a one-time service. When you work with me, my goal is to build a lasting relationship with both you and your dog. I love watching my clients and their dogs progress over time, and I am always happy to continue supporting them as new goals and challenges come up.

When I'm not working with other people's dogs, I'm usually with my own two: Sandler, my Italian Greyhound, and Hoppy, my rescue Pit Bull. They each bring their own personalities, quirks, and lessons to my life, and I wouldn't have it any other way.

Whether you are welcoming a new puppy into your family, working through a behavioral challenge, or simply looking to strengthen the relationship you have with your dog, I'm here to help. My goal is not just to train your dog, but to help you better understand each other, communicate more clearly, and build a relationship that lasts.

I can't wait to meet you and your dog!

Brittany Mead Canine Coaching
Client Service Agreement and Adult Liability Release
Florida services | Form version FL 2026 10 11
This agreement sets the terms for the canine services identified in the attached Service Schedule. It includes
care instructions, client responsibilities, financial terms, emergency permissions, and a release of certain legal
claims belonging to the adult signer.

READ SECTION 11 CAREFULLY. IT RELEASES CERTAIN CLAIMS ARISING FROM THE PROVIDER'S OWN
ORDINARY NEGLIGENCE. THE EXCLUSIONS IN THAT SECTION APPLY THROUGHOUT THIS AGREEMENT.

1 Parties and authority
Provider legal name: _____________________________________________________
Doing business as Brittany Mead Canine Coaching
Provider business address: _________________________________________________
Provider notice email and telephone: __________________________________________
Client legal name: ________________________________________________________
Client address: __________________________________________________________
Client email and telephone: _________________________________________________
Effective date: __________________ Service Schedule reference: __________________
The person or legal entity identified above is the "Provider." Each adult owner who signs as Client is a party
only for that person's own rights and obligations. The "Dog" means each dog identified in a signed Schedule A.
Client is at least 18, has legal capacity, and is an owner or has documented authority to authorize the listed
care. Identify all owners and any limited authority in Schedule A. One signature does not release another
owner's or person's independent rights.

2 Covered services and duration
This agreement applies only to the bookings, dogs, dates, locations, and services identified in Schedule B and
any later signed service addendum expressly incorporating this form version. Services may include private or
in-home training, board and train, boarding, and approved ground transport. Other activities require the
permissions in Schedule B. A future booking is not covered merely because this form was signed once; its
written confirmation must incorporate this agreement and confirm updated disclosures and permissions.
Schedules A through D, attached before signing, are part of this agreement. Complete a separate Schedule A
and C for each dog. Mark inapplicable fields "N/A." Blank, conflicting, or unclear permissions grant no authority
until clarified in writing. Blank additional-fee fields create no unspecified charge. All signing owners must
accept one common set of care instructions and permissions for each dog. The designated primary contact
may act only within the accepted authority and spending limits. Provider must accept the booking in writing
before it is reserved.

3 Care plan and assigned caregivers
Provider will discuss the goals, material restrictions, expected service structure, and proposed methods with
Client. Provider agrees to use reasonable care and comply with applicable law. Sections 10 and 11 allocate
certain civil risks; they do not authorize unsafe or unlawful conduct. Provider may adapt ordinary exercises to
the Dog's condition and progress within the agreed scope. Material changes in location, care arrangements,

                                                                                     Form FL 2026 10 11 | Page 1 of 14

cost, or a specifically consented method require Client's written agreement, subject to the limited emergency
authority below.
Schedule B identifies the primary care address, authorized caregivers, backup arrangements, overnight
staffing, and expected unattended intervals. Only Brittany Mead and the identified authorized personnel may
routinely provide care. An independent veterinary clinic is not the Provider. Provider will not transfer routine
custody to an undisclosed boarding operator or substitute caregiver without consent. Necessary emergency
transfers are governed by Sections 12, 13, and 20.

4 Medical and care disclosures
Before services, Client must complete Schedule A accurately to the best of Client's knowledge after reasonable
inquiry, supply relevant veterinary instructions, and promptly report changes before the next visit or custody
transfer. Disclose illness or infectious exposure, allergies, injuries, surgery, mobility restrictions, seizures,
breathing or heart problems, chronic conditions, parasites, medications, supplements, special diets, and
handling limitations. Disclose intact status, heat cycles, pregnancy or suspected pregnancy, and any breeding-
related restrictions. Provider does not offer breeding or reproductive management unless a separate lawful
service agreement expressly provides otherwise.
Client must provide legally required rabies documentation or a valid veterinary exemption where permitted,
and meet the written admission requirements agreed before booking. Additional vaccines, parasite
prevention, testing, or veterinary clearance may be business admission conditions, but are not represented
here as universally required by law. Provider may postpone admission or seek veterinary guidance if records or
suitability are uncertain. Vaccination and precautions do not eliminate every risk of illness.
Client supplies sufficient labeled food and any accepted medication in its original labeled container, with
current written dosage, timing, and veterinarian instructions. Provider will administer only medications and
tasks it has expressly accepted and is legally permitted and able to perform. Provider will not independently
change a prescription, diagnose, or prescribe. Injections, controlled drugs, sedation, complex medical care, or
other specialized tasks require separate written acceptance and appropriate veterinary direction.

5 Behavior and legal restrictions
Client must disclose every known bite, attempted bite, injury to a person or animal, aggression, resource
guarding, escape attempt, severe separation distress, travel panic, destructive behavior, handling sensitivity,
and relevant adverse response to training. Disclose incidents even if believed to have been provoked, minor,
accidental, or resolved. Provide the circumstances, approximate dates, treatment required, triggers, and
management measures. If history is unknown, say so rather than stating that no incidents occurred.
Client must disclose any dangerous-dog designation, animal-control investigation, quarantine, rabies exposure,
impoundment, court order, insurance restriction affecting the planned care, or other legal limitation, and
provide supporting documents. Neither acceptance into training nor this agreement removes a legal
restriction. Provider may decline a dog whose needs or risks exceed available facilities, staffing, qualifications,
or insurance. Disclosure enables assessment; it does not promise acceptance or a behavioral cure.

6 Training methods and safety rules
Provider will explain proposed methods, material risks, and reasonable alternatives before obtaining any
required specific consent. Routine humane handling and agreed leash, harness, collar, reward, crate, or
environmental-management exercises are limited by the care plan. Electronic or stimulation collars, prong or
other pressure-based training tools, planned direct dog interactions, off-leash work, water activities, and
routine transport require the corresponding written choice and details in Schedule B. Silence, an unchecked
box, or general agreement to training is not specific consent.

                                                                                    Form FL 2026 10 11 | Page 2 of 14
Page 3 of 14
Brittany Mead Canine Coaching

Equipment can create pressure, discomfort, skin irritation, entanglement, stress, fear, or an adverse behavioral
response. Provider will use appropriate fitting, inspection, supervision, duration, and stop criteria for the
method actually agreed. No permission authorizes abusive handling, illegal conduct, or disregard of veterinary
restrictions. Provider may use reasonable, proportionate emergency restraint or separation to prevent
imminent harm, with prompt notice when practicable.
Client must follow reasonable safety instructions, use agreed equipment as taught, supervise the Dog outside
Provider's custody, and promptly report concerns. Client may withdraw an optional method prospectively in
writing. Provider will stop it as soon as safely practicable and discuss an alternative, revised scope, or
termination with the accounting required by Section 17. Client consent does not override leash, public-access,
quarantine, or other legal rules.

7 Boarding and board and train care
Boarding is offered only for dogs Provider has trained or is currently training, unless Provider expressly agrees
otherwise in Schedule B. Sleeping arrangements, food, exercise, separation, handling, and medical tasks must
fit the written care plan. Boarding does not include continuous individual observation or on-site overnight
attendance unless Schedule B expressly promises it. The actual staffing and unattended intervals must be
disclosed before acceptance; this sentence does not permit an unsafe arrangement.
Dogs from different households will not have planned direct contact or shared group play without specific
written permission from the affected owners and Provider's suitability assessment. Permission may be
restricted or revoked. Shared surroundings can still involve noise, odors, indirect exposure, or proximity.
Known incompatible dogs and intact dogs requiring reproductive separation must be managed accordingly.
Provider may separate a dog, pause activities, seek veterinary care, or arrange safe return if stress, illness,
aggression, or another condition makes continued care unsuitable.

8 Transport and custody handoffs
Routine ground transportation is limited to the purposes, destinations, dates, drivers, and arrangements
selected in Schedule B. Client must disclose travel illness, panic, escape behavior, restraint difficulties, and
relevant health limits. Provider will use appropriate restraint or a secured enclosure, ventilation and
temperature precautions, and safe loading and unloading. Routine sedation is not authorized by transport
consent. Appropriate veterinary treatment is governed separately by Section 12.
Custody begins when Provider or an identified authorized caregiver affirmatively accepts control of the Dog at
the agreed handoff. It ends when Client or a listed adult recipient accepts control. An unattended drop-off, an
unanswered message, or release into a yard does not complete a handoff unless a separate signed
unattended-access procedure expressly covers it. Provider may verify the recipient's identity and refuse an
apparently unsafe release. Traffic, weather, safety, and authorized emergency care may require reasonable
route or timing changes, with notice when practicable. Interstate or air transport requires a separate
agreement.

9 Home access and client premises
For home visits, Client authorizes access only to the address, areas, dates, and entry method recorded in
Schedule B and represents having authority to grant it. Client will disclose known hazards, unsafe fencing,
pools, unsecured animals, weapons accessible in the work area, and relevant recording arrangements, and will
secure unrelated animals and valuables. Access codes should be supplied securely rather than written on
general copies of this agreement. Provider will reasonably safeguard access devices and return them or
relinquish access when services end. This permission does not authorize access to unrelated private areas or
use of property for unrelated purposes.

                                                                                      Form FL 2026 10 11 | Page 3 of 14

10 Acknowledgment and assumption of service risks
Dogs are living animals whose behavior and health can change unexpectedly, including when previously
friendly, trained, vaccinated, or screened. I understand the following risks may occur in the services I
authorize, even when reasonable care is used:
 Bites, scratches, fights, jumping, pulling, falls, collisions, or other contact affecting me, the Dog, another
    animal, or property.
 Escape, becoming lost, equipment failure, entanglement, choking, ingestion of foreign material, damage to
teeth or nails, or injury in a crate, kennel, yard, vehicle, or training location.
 Contagious disease, parasites, allergic reactions, stress-related illness, aggravation of a preexisting or
undisclosed condition, reproductive complications, or other sudden medical events.
 Traffic incidents during authorized travel or loading; heat, storms, flooding, wildlife, insects, unfamiliar
surfaces, and other environmental hazards; and drowning or water injury if water activity is specifically
    authorized.
 Pain, severe or permanent injury, disability, death, veterinary or medical costs, loss of the Dog, or damage
to other property. Training may not eliminate aggression, escape behavior, or other risks.
I have had an opportunity to discuss risks particular to my Dog and the selected services. I voluntarily accept
the inherent risks of those services. This acknowledgment is distinct from the ordinary-negligence release in
Section 11 and does not expand that release or its stated exclusions.

11 Express release of ordinary negligence claims
TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, I RELEASE THE PROVIDER IDENTIFIED IN
SECTION 1, BRITTANY MEAD, AND THE PROVIDER'S EMPLOYEES AND THE AUTHORIZED CAREGIVERS
IDENTIFIED IN SCHEDULE B, WHILE ACTING WITHIN THEIR ASSIGNED SERVICE DUTIES, FROM CLAIMS
THAT I PERSONALLY OWN FOR MY BODILY INJURY OR DEATH, OR INJURY TO, ILLNESS OF, DEATH OF,
ESCAPE OF, LOSS OF, OR DAMAGE TO MY DOG OR OTHER PROPERTY, ARISING FROM THE SERVICES I
AUTHORIZE UNDER THIS AGREEMENT.
THIS RELEASE EXPRESSLY INCLUDES CLAIMS CAUSED IN WHOLE OR IN PART BY A RELEASED PARTY'S
OWN ORDINARY NEGLIGENCE, MEANING A FAILURE TO USE REASONABLE CARE. IT INCLUDES
ORDINARY NEGLIGENCE IN ANIMAL HANDLING, SUPERVISION, TRAINING, BOARDING, ROUTINE CARE,
EQUIPMENT, PREMISES USED FOR THE SERVICES, AND AUTHORIZED GROUND TRANSPORTATION,
INCLUDING LOADING AND UNLOADING.
This release does not apply to gross negligence, reckless or intentional misconduct, fraud, animal cruelty,
conduct outside the authorized service scope, or a claim, duty, or liability that applicable law does not
permit to be released. It does not waive an independent claim of a minor or any nonsigning person. It does
not excuse payment or refund obligations, failure to supply purchased services, or another contractual
obligation unrelated to the injury or damage released above. These exclusions control over every other
provision of this agreement.
The release applies only to my own rights and, to the extent legally permitted, persons asserting those same
rights through me or my estate. It does not represent that I can release another person's independent rights. I
understand that I may be unable to recover covered losses caused by ordinary negligence. I have been able to
ask questions and choose whether to accept these terms.

12 Emergency veterinary authorization
Before any custody service, Client must complete Schedule C for each dog. Client appoints Provider as a limited
agent to arrange veterinary evaluation, transport, and the care specifically authorized there when Provider
reasonably believes prompt attention is needed. Provider will make reasonable efforts to contact Client, then
the designated alternate, without delaying necessary transport or protective steps. The alternate may approve
treatment or spending only to the extent expressly selected in Schedule C. An emergency contact is not
automatically an unlimited decision-maker.
Subject to the facility choice recorded in Schedule C, Provider may use another available licensed veterinarian
or emergency facility if the preferred facility is unavailable or delay would be inappropriate. A declined choice
does not grant contractual authority to use that alternative; any independent authority supplied by law
remains unaffected. The veterinarian makes clinical recommendations. Examination, diagnostics, stabilization,
and additional treatment are subject to the completed spending limits and emergency election. A blank
amount or unsigned election does not authorize unlimited spending. Incomplete instructions do not prevent
seeking emergency assistance permitted by law, but do not create unrestricted treatment or financial
authority.
Client authorizes the exchange of records reasonably necessary for the Dog's care and will arrange direct
payment to the veterinary facility where possible. Client will pay or reimburse actual, documented, authorized
veterinary, transport, and related emergency costs. Provider is not required to advance its own money or
guarantee credit or a facility's acceptance. Payment responsibility and permission to obtain care do not decide
ultimate legal fault or waive any claim preserved by Section 11 or applicable law.
This agreement does not authorize Provider to choose euthanasia, transfer ownership, surrender the Dog for
adoption, authorize breeding or elective sterilization, or approve other elective procedures. Otherwise-
authorized emergency treatment may include medically necessary surgery that incidentally causes sterility,
within the selected treatment and spending limits. A veterinarian or public authority may exercise authority
independently supplied by law. Existing veterinary advance instructions must be provided and accepted by the
treating facility; this agreement does not create a veterinary resuscitation order.

13 Incident response and communications
Provider will notify Client as soon as reasonably practicable of a material injury, illness, escape, bite,
emergency transfer, or other significant event involving the Dog. Immediate protective action may come first.
Provider may document the event, seek veterinary or animal-control guidance, contact emergency services,
and comply with lawful reporting, quarantine, evidence-preservation, or public-health requirements. For an
escape, Provider will undertake reasonable safe recovery efforts and notify appropriate contacts. No particular
recovery outcome is guaranteed.
Client must maintain working contact information, remain reasonably reachable, and designate an adult who
can accept the Dog if necessary. Routine photo or text updates occur as reasonably agreed and are not
continuous monitoring. Client should promptly report a concern or post-stay illness so it can be addressed;
failure to report within a privately imposed short deadline does not automatically extinguish an otherwise valid
claim.

14 Third party claims and limited indemnity
Client remains responsible for duties imposed on Client as a dog owner and for Client's own acts and
omissions. Neither this contract nor the Dog's participation eliminates applicable ownership, custody,
enclosure, quarantine, or public-safety duties. A claim by a neighbor, visitor, other client, driver, or other
nonsigning person is not released by this agreement.

                                                                                     Form FL 2026 10 11 | Page 5 of 14
Page 6 of 14
Brittany Mead Canine Coaching

Client will indemnify and reimburse Provider and the other released parties for third-party damages and
reasonable defense expenses only to the extent caused by Client's negligent or intentional act or omission,
material misrepresentation, failure to disclose known material information required here, or material violation
of an agreed safety obligation. The obligation excludes the portion caused by any released party's negligence
or other misconduct. It creates no immediate duty to defend based on an allegation alone.
Responsibility and amount must be established by a final judgment or a written settlement approved by Client,
with approval not unreasonably withheld. Provider must give reasonably prompt notice and a reasonable
opportunity to participate in the defense, subject to insurer rights. No settlement may admit Client's fault or
impose a nonmonetary obligation without Client's written consent. This provision does not waive a third
party's rights or require reimbursement merely because a minor exercises a protected claim.

15 Training outcomes and service limits
Provider will supply the agreed instruction and care but does not promise a particular behavioral result,
completion date, permanent change, cure for aggression, safe unsupervised interaction, or reliable recall in
every situation. Progress depends on health, temperament, history, environment, practice, and consistent
handling. Client remains responsible for ongoing supervision and lawful management after sessions or return
of the Dog. A training result does not replace necessary containment or medical care.
Service-dog or therapy-dog foundations and Canine Good Citizen preparation do not themselves guarantee
legal status, public-access rights, certification, suitability for a particular person, or a passing examination.
Veterinary diagnosis and treatment, protection or bite work, breeding, and services outside Schedule B are
excluded unless a separate lawful agreement expressly adds them.

16 Prices payment and additional costs
Schedule B records the actual price, included services, dates, package allocation, deposits, payment due dates,
and any separately agreed charges. Unless Schedule B states otherwise, payment for private training is due at
the first session; board-and-train payment is one-half on the first day and the remainder on the last day.
Boarding and transport payment timing must be recorded in Schedule B. Provider will provide receipts and an
itemized account on request. No recurring charge, automatic renewal, stored-card authority, or unspecified
late fee is created by this agreement.
An extension, added service, equipment purchase, special cleaning, or third-party expense requires prior
written approval of its amount or method of calculation, except expenses within express emergency authority.
Client is responsible for documented physical damage directly caused by the Dog beyond ordinary wear, to the
extent not attributable to Provider's fault. Charges must reflect reasonable repair cost or depreciated
replacement value, as appropriate, with supporting detail. Ordinary care costs cannot be relabeled as damage
charges.

17 Cancellation rescheduling and refunds
The booking-specific cancellation deadline and charge, if any, must be completed in Schedule B before signing.
A cancellation or reservation charge must be clearly described, reasonable for the booked commitment and
expected loss, and lawful. If no charge is completed and agreed, none applies. An identified deposit is credited
against the price; calling it a deposit does not make it automatically forfeitable. There is no automatic
forfeiture of the entire unused package merely because one service has begun.
Completed services are earned at the prices allocated in Schedule B, subject to remedies that applicable law
preserves. On cancellation, early departure, safety termination, or another early end, Provider will issue an
itemized account of services actually supplied, approved equipment or third-party costs, and any applicable
agreed cancellation charge, and refund the remaining unearned prepaid balance within 14 calendar days.

Package units will not be retroactively repriced at a higher undisclosed rate. No overlapping charges may
compensate the same loss twice.
If Provider cancels for its own convenience, or cannot supply a purchased service for reasons not materially
caused by Client's breach, Client may choose mutually agreed rescheduling or a refund for the unprovided
portion without a cancellation or reservation charge. A termination materially caused by Client's breach
remains subject only to the lawful, specifically agreed charges and accounting above. Weather, illness, and
emergency interruptions are subject to Section 20. This section preserves mandatory cancellation rights,
consumer remedies, and lawful disputes of unauthorized or incorrect payments.

18 Safety suspension and termination
Provider may pause, modify, decline, or end services if illness, aggression, escape risk, a material undisclosed
history, unsafe premises, inability to meet the Dog's needs, Client's material noncompliance, or a comparable
specific concern makes continued work inappropriate. Provider will explain the reason and coordinate a
reasonably safe return to Client or an authorized adult, or a necessary lawful emergency transfer. Provider will
not continue an unsuitable method solely to complete a prepaid package.
Client may end future services by notice, subject to the completed cancellation terms and safe pickup
arrangements. Ending services does not cancel earned fees or authorized expenses, expand the release, or
erase an existing preserved claim. Provider will not expose or abandon the Dog, deny necessary care, or use a
payment dispute as authority for an otherwise unlawful retention or ownership transfer.

19 Late pickup and unclaimed dogs
Client must collect the Dog on time or obtain an approved extension. Agreed extra care charges appear in
Schedule B; urgent necessary costs must be reasonable, documented, and subject to the agreed rates, expense
permissions, and emergency spending limits. If Client is unreachable, Provider may contact the designated
alternate and appropriate animal-welfare authorities. Any abandoned-animal procedure must follow
applicable law, including required qualifications and notice. Late pickup alone does not transfer title or
authorize Provider to sell, adopt out, surrender, or euthanize the Dog. This agreement creates no independent
ownership-transfer or lien-enforcement procedure.

20 Weather disasters and unavoidable interruptions
Provider may shorten, relocate, postpone, or cancel activities because of dangerous heat, severe weather,
flooding, evacuation, illness, utility failure, or other conditions that make the planned service unsafe or
unavailable. For a Dog already in custody, Provider may undertake necessary safe evacuation or temporary
transfer to a reasonably suitable lawful refuge if a named backup is unavailable. This physical-safety authority
does not itself authorize spending above the agreed limits; Provider will seek Client, authorized alternate, or
public assistance for excess costs. Veterinary care remains subject to Schedule C. Provider will take reasonable
precautions, mitigate avoidable loss, and communicate as soon as practicable.
An event beyond a party's reasonable control excuses only the performance actually prevented and only for
the necessary period. It does not excuse reasonable emergency preparation, care of an animal in custody,
compliance with lawful orders, or accounting for unused prepaid services. Approved reasonable relocation
costs are itemized separately from veterinary treatment and remain subject to the selected authorization
limits.

21 Records privacy and optional publicity
Provider may maintain service, health, behavior, payment, incident, and consent records reasonably needed to
deliver care, communicate with Client, comply with law, resolve a claim, or work with a veterinarian or insurer.

                                                                                   Form FL 2026 10 11 | Page 7 of 14

Records must be reasonably safeguarded and disclosed only for these purposes or with separate permission.
Client will receive a copy of the signed agreement and may request relevant service and incident records,
subject to another person's privacy and applicable law.
Schedule D separately addresses private dog training images and optional marketing use. Publicity is not a
condition of receiving services and does not affect price. This agreement gives no general permission to publish
a person's likeness, voice, children, private communications, home address, or confidential information. It
contains no restriction on truthful reviews or lawful complaints. Recording audio or other people requires
whatever separate consent applicable law requires; dog media permission does not supply it.

22 Adult participation and minors
Each adult who handles a dog or participates in training must receive the safety instructions and sign an
appropriate agreement for that person's participation. Client must not represent that another person is bound
merely because that person is a family member or guest. Client must obtain advance approval for a minor's
participation and provide appropriate adult supervision. Provider may limit participation for safety.
This adult agreement does not release a minor's personal-injury, death, or independent property claim, and
does not constitute a Florida statutory minor waiver. Any proposed minor participation release must be a
separately prepared, appropriate Florida document. The absence of such a release does not grant permission
for unsupervised or unapproved participation.

23 Florida law disputes and agreement terms
Florida law governs to the extent applicable, without displacing any controlling mandatory law. Subject to a
legally required alternative venue, an action concerning this agreement may be filed in a court with jurisdiction
in the Florida county where the principal booked services occurred, or the federal court serving that county.
Either party may seek voluntary mediation. Small-claims proceedings, emergency relief, regulatory reports,
and mandatory legal rights are not conditioned on mediation. This agreement does not impose arbitration,
waive a jury, or shorten a statutory filing period. Each party bears its own legal fees unless applicable law or
the limited third-party indemnity permits recovery.
The completed agreement and attached schedules state the agreed terms. An advertisement, automated
assistant response, or later website revision does not silently change them. A material amendment requires
both parties' written agreement and must identify the affected booking and form version. A new risk, method,
dog, or service must be addressed before it begins. A waiver of one breach is not a waiver of another.
If a provision is unenforceable, it is severed only to the extent the law permits and the remaining terms can
operate consistently with the agreed protections and exclusions. Severability does not enlarge a release,
create a missing consent, or authorize rewriting an unlawful term beyond what a court may lawfully do. No
provision is a waiver of a nonwaivable right.
The parties may choose paper signatures or mutually agree to electronic execution. Electronic execution must
evidence the signer's identity and intent and preserve the exact accepted agreement and schedules in a form
the signer can retain. Client may request paper execution for future transactions without invalidating a
completed transaction. Notices use the contacts in Section 1 unless updated in writing; this does not substitute
ordinary email for a special delivery method required by law.

itself authorizing excess charges. Additional approval or independent legal authority is needed for excess spending.
Veterinary costs are controlled by Schedule C. A blank field is not unlimited authority.
I understand the disclosed risks and limits. I may withdraw optional permission prospectively under Section 6. No
selection authorizes unlawful activity, animal cruelty, or disregard of agreed health restrictions.

Schedule C Emergency veterinary authorization
Complete one for each dog before custody. Confirm the receiving clinic will accept this authorization.
Client: ____________________________ Dog: _________________________________
Primary phone: _____________________ Alternate phone: ________________________
Alternate adult decision-maker name and phone: __________________________________
Alternate may approve treatment and spending if I am unavailable: [ ] Yes [ ] No
Alternate TOTAL per-incident spending limit if Yes: $____________
This ceiling includes prior spending. If Yes is selected and the amount is blank, the standard limit controls. Only an
expressly higher amount allows the alternate to approve more; other treatment restrictions remain. Resolve conflicting
instructions before custody.
Other limits on alternate authority: ____________________________________________
Preferred veterinarian and phone: _____________________________________________
Preferred emergency facility and phone: ________________________________________
Other available licensed veterinary facility if needed: [ ] Authorize [ ] Decline
If declined, agree on workable emergency arrangements before custody. Provider may decline the booking.

Treatment authority and monetary limits
I authorize veterinarian-recommended examination, diagnostics, and stabilization, including emergency
transport, within this total per-incident limit:
Standard total limit per incident: $________________ Client initials: ______________
If a veterinarian advises that delay risks imminent death or serious lasting harm and neither I nor my
authorized alternate is reachable, select one:
[ ] Authorize urgently necessary treatment up to a TOTAL per-incident ceiling of $________________, including
prior spending. This ceiling must equal or exceed the standard limit. Client initials: __________
[ ] Require my or my authorized alternate's approval above the standard limit. I understand treatment may be
limited or delayed. Client initials: __________
Resolve blank standard limits or conflicting amounts before custody. Without a valid emergency election, the standard
limit controls. No blank authorizes unlimited spending. Provider and the clinic need not extend credit.

Clinical directions and payment arrangements
Critical conditions allergies and restrictions or attached care-plan reference: ______________
Existing veterinary advance instructions attached: [ ] Yes [ ] No
Payment arrangement and payer or insurer contact: _________________________________
Arrange payment directly with the clinic; do not put card numbers here. I authorize necessary record sharing and actual
authorized expenses under Section 12. Payment does not decide ultimate legal fault.
No authority to choose euthanasia, breeding, elective procedures, or ownership transfer is granted. Authorized
emergency surgery may incidentally cause sterility, within agreed limits. Independent authority supplied by law
remains unaffected.

Schedule D Image permissions and private updates
Client: ____________________________ Dog or dogs: __________________________
These permissions concern images of the Dog. They do not authorize recording or publication of a person's
face, voice, children, private home details, address, or private communications. Separate consent is required
where appropriate. Neither permission supplies consent to audio recording of other people.

Private care and training images
[ ] Authorize [ ] Decline Dog photographs or video for private training records and direct updates to me.
Reasonable incident documentation needed for care, insurance, legal compliance, or a dispute may still be
retained under Section 21 with appropriate access limits.
Limits on private images: ____________________________________________________

Optional marketing permission
[ ] Authorize [ ] Decline Provider may use selected images of my Dog on its own website, social accounts,
printed promotional materials, or educational examples. This is a nonexclusive permission for the selected
purposes without payment to me. It is optional and does not affect eligibility for services or the service price.
Use the Dog's first name publicly: [ ] Yes [ ] No
Additional restrictions: _____________________________________________________
A blank or inconsistent marketing choice means no permission. Permission does not authorize selling the
images for unrelated stock or data uses, fabricating an endorsement, misleading edits, or public disclosure of
my personal information. Provider will not identify a child or other person under this permission.
I may withdraw permission for new uses by written notice. Provider will stop creating new uses and make
reasonable efforts to remove materials under its direct control. Previously distributed print materials and
third-party copies may not be recoverable. Withdrawal does not require deletion of lawfully retained private
care or incident records.

24 Final acknowledgments and signatures
Before signing, confirm the legal Provider name and all service, fee, health, care, spending, and permission
fields. Every attachment must be present and identified. Each additional owner must initial Section 11 and sign
a counterpart or additional signature page expressly identifying this agreement and the same completed
schedules. Each signature binds only rights the signer can lawfully agree to release. Resolve conflicting
instructions before the affected service begins.
I have read Sections 1 through 24 and the completed Schedules A through D. I understand the separate
ordinary-negligence release in Section 11 and the limited third-party indemnity in Section 14. I had sufficient
opportunity to ask questions and obtain advice. I sign voluntarily, intend to be bound, and will receive a
complete copy. No one has promised that my signature waives a child's or nonsigning person's independent
claim.
